Due to expansion of our Pharmacy Team, an exciting opportunity has arisen for a 2 days per week part-time (0.4FTE) Medicines Management Pharmacist to join us in supporting healthcare professionals and operational colleagues to ensure optimal medicines management across the organisation.

Change Grow Live is a leading national health and social care charity, working with individuals, families and communities affected by substance misuse, crime, homelessness, domestic abuse, and anti-social behaviour.

You will have an active role in supporting services to make the best use of medicines in a safe, cost-effective, high-quality, and evidence-based manner. Your role will include analysing prescribing data, supporting adherence to our prescribing formulary, developing and reviewing guidance documents, providing medicines information to our colleagues working in our services, and helping them to comply with regulatory requirements. You will represent the Pharmacy Team at key meetings and contribute to the delivery of the Pharmacy Team (and the wider Clinical and Care Team) strategy.

You will be able to communicate effectively with a wide range of healthcare professionals and stakeholders to deliver pharmacist support and medicines management expertise.

This post is mostly home-based, however there is a requirement to visit community services as needed, and the occasional face-to-face meeting.
